New Patient Information
Purpose Pediatrics is a walk-in clinic, which means no appointment or advance registration is required. If this is your child's first visit, the information below will help you understand what to bring, what to expect, and how we coordinate care with your child's regular pediatrician.
What to Bring
- Your child's insurance card
- Photo ID for the parent or guardian
- A list of current medications and any known allergies
- The name and contact info of your child's regular pediatrician (if established)
- Your preferred pharmacy name and location
First-Visit Forms
When you check in, we'll have a short intake form for new patients. It covers your child's medical history, allergies, current medications, and emergency contact info.
About Your Visit
We're built for after-hours acute pediatric illness — not preventive care or long-term condition management. That means:
- We treat sick visits and minor injuries
- We do not provide well-child visits, vaccinations, or long-term care
- Any child is welcome regardless of pediatrician
- Any child is welcome regardless of vaccination status
- No referral needed
- No requirement to be an existing patient
If your child needs ongoing care, we'll recommend follow-up with their regular pediatrician.
If Your Child Doesn't Have a Regular Pediatrician
We treat all children regardless of whether they have an established pediatrician. Following the visit, you will be given a written summary documenting the evaluation, diagnosis, and any treatment provided. This record can be shared with a primary care provider in the future. If you need a referral to a local pediatrician, please let our team know at check-in.
After Your Visit
You'll leave with a written summary of the visit, any prescriptions sent to your pharmacy of choice, and a clear plan for what to watch for and when to follow up.
