What's Included in a Sports Physical
A standard pediatric sports physical at Purpose Pediatrics includes:
- Review of your child's medical history
- Vitals (height, weight, blood pressure, pulse)
- Heart and lung exam
- Musculoskeletal evaluation
- Vision screen
- Completion and signing of the school or league physical form
If we identify anything that needs further evaluation, we'll let you know and recommend follow-up with your child's regular pediatrician.

- Your child's blank sports physical form. Our office also has blank forms available.

No. Walk in during our open hours. Feel free to call and ask the expected wait time.
Can my child get a dental clearance physical here?
Yes. We provide dental clearance physicals. We can bill your insurance or it’s $50 self-pay .
Will you sign my child's school or league form?
Yes, as long as your child clears the exam. If we find anything that warrants further evaluation, we'll explain what needs to happen before clearance can be signed or refer your child’s regular pediatrician.
